Integrating advance nursing practice to meet the complex demands of Residential Aged Care
There are more older people with advanced frailty receiving care at home than ever before. This positive change has resulted in older people entering residential aged care (RAC) at an older age and with much higher dependency and care needs. RAC facility nurses and caregivers are crucial for early identification and intervention when a resident’s health deteriorates and for those with complex palliative care needs. Yet, RAC staff seldom have contact with geriatricians or gerontology nurse specialists and often do not have the role modelling and support necessary to improve their clinical knowledge and assessment skills. However, this has begun to shift over the last decade. Many models of care now employ gerontology nurse specialists and nurse practitioners to support older people and the staff who care for them. This presentation will describe several successful integrated models of care and their outcomes as well as discuss potential future RAC advanced nursing care trends.
